Telehealth Informed Consent
Effective date: October 30, 2026
This consent explains what it means to receive care through telehealth on Wilde Aesthetics, a brand of the DynaWellness network operated by DynaWell Peptides Inc. Your care is provided by independent licensed healthcare providers and their affiliated medical practices, whose own informed consent for telehealth services also applies. By checking the consent box during checkout, you acknowledge that you have read, understood, and agree to the following.
1. What telehealth is
Telehealth is the delivery of healthcare services using electronic communications, where you and your provider are not in the same physical location. On this platform, care is primarily asynchronous: you complete a structured health questionnaire, and an independent licensed provider reviews it — together with any follow-up questions, identity verification, and messages — to evaluate whether treatment is appropriate. Where clinically required or requested, a synchronous (phone or video) consultation may be arranged.
2. Who provides your care
Your care is provided by independent healthcare providers licensed in your state. DynaWell Peptides Inc operates the technology platform and the storefront but does not practice medicine and does not influence clinical decisions. Providers are never compensated based on whether they prescribe.
3. Expected benefits
- Convenient access to licensed clinicians without travel or scheduling.
- Care on your schedule — questionnaires and messaging on your own time.
- Continuity: your treatment is re-evaluated at each refill and you can message your care team at any time from the patient portal.
4. Possible risks and limitations
- The provider does not perform an in-person physical examination and relies on the information you provide. Incomplete or inaccurate answers can lead to inappropriate care.
- Asynchronous review may involve delays in communication compared to an in-person visit.
- In rare cases, technical failures could delay or disrupt care. Electronic systems also carry inherent privacy risks despite safeguards.
- Telehealth may not be appropriate for your condition; the provider may determine that in-person care is required and decline to treat you remotely.
5. No guarantee of prescription
Paying for a consultation is not a purchase of medication. Prescriptions are issued only if the reviewing provider determines a treatment is safe and appropriate for you, in their sole professional judgment. If treatment is declined, your order will be refunded per the Terms of Service.
6. Compounded medications
Some treatments that may be prescribed through this platform, including certain GLP-1 medications (such as compounded semaglutide or tirzepatide), are compounded by a licensed U.S. pharmacy for an individual patient pursuant to a valid prescription. Compounded medications are not FDA-approved, and the FDA does not verify their safety, effectiveness, or manufacturing quality. Brand-name FDA-approved alternatives may be available. By consenting, you acknowledge that you have been informed of this distinction.
7. Alternatives
You are free to seek care in person from a physician, clinic, or hospital of your choice at any time, instead of or in addition to telehealth.
8. Emergencies
Telehealth on this platform is not for emergencies. If you experience a medical emergency — including but not limited to chest pain, difficulty breathing, signs of stroke, severe allergic reaction, or thoughts of harming yourself or others — call 911 or go to the nearest emergency room immediately. For mental-health crises, you can also call or text 988 (Suicide & Crisis Lifeline).
9. Your responsibilities
- Answer all health questions truthfully and completely, and update your care team if your health changes.
- Verify your identity when requested (government-issued photo ID). Care cannot proceed without successful identity verification where it is required.
- Read all medication guides and follow the prescribing provider’s directions.
- Report side effects to your care team promptly through the patient portal.
10. Privacy of your health information
Your health information is transmitted securely to the providers and clinical network responsible for your care and is protected as described in our Privacy Policy and HIPAA Notice. Messages with your care team travel through the secure patient portal.
11. Withdrawing consent
You may withdraw this consent at any time by stopping use of the Services and notifying us at telemed@wildeaesthetics.com. Withdrawal does not affect care already provided, and standing legal or clinical record-keeping obligations survive withdrawal.
12. Acknowledgment
- I understand how telehealth works, including its benefits, risks, and limitations.
- I understand that my care is provided by independent licensed providers, not by the platform operator.
- I understand that no specific treatment or prescription is guaranteed.
- I understand that compounded medications, if prescribed, are not FDA-approved.
- I consent to receiving care through telehealth and to being contacted about my treatment by my care team.
